Can you home-office without a nanny?

I'm looking for anyone that has successfully run a home office with a baby or two on their hip.

At time of posting this thread, we have a nine-month old girl and a second baby due in November. Their age difference will be just shy of 14 months apart. I'm a co-sleeping, baby carrying, nursing momma, and am challenged. There was the idea of what I would be able to accomplish with a baby (soon two babies) on my hip, and then the reality of what goes undone... whether it be showering for me, mopping the kitchen floor, or, sadly, starting my new law practice from home. I've decided to not work for a law firm, since so many, with good reason, have minimum billable hours for new associates. As I need the flexibility to be able to tend to my babies and my aging parents, working in a traditional setting is not for me, or my family.

Is the answer that I need in-home care such as a sitter or nanny, if so, any suggestions on how to go about finding someone that is compatible to our parenting style? Or, am I missing the magic something or other that will soothe and entertain my babies while their Maman gets some work done?
